Compassionate, Specialized Support for Memory Loss at Home
A diagnosis of Alzheimer’s or dementia affects the entire family. Navigating the changes in behavior, safety, and communication can be overwhelming, but you don’t have to do it alone. At Northwest Home Health & Rehab, our Memory Care Program is designed to support both the patient and the caregiver, allowing loved ones to age in place safely and with dignity.
We provide a structured, clinical approach to memory care that focuses on preserving ability and reducing anxiety.
Your Care Outcomes:
- Prolonged Independence: By establishing consistent routines and utilizing cognitive stimulation techniques, we help patients maintain their daily skills and connection to the world for as long as possible.
- Enhanced Home Safety: We conduct rigorous safety evaluations to reduce risks associated with wandering, falls, and kitchen safety, ensuring the home environment supports—rather than hinders—care.
- Management of Behavioral Changes: Our team provides strategies to manage complex symptoms such as sundowning, aggression, or confusion, reducing the need for psychotropic medications or hospitalization.
- Caregiver Relief & Education: We care for the family, not just the patient. We provide hands-on training for family caregivers, teaching effective communication strategies to reduce frustration and prevent caregiver burnout.
Our Specialized Approach:
Our multidisciplinary team works together to create a calm, supportive environment:
- Skilled Nursing: Monitors medication compliance, addresses underlying medical issues (like UTIs) that can worsen confusion, and coordinates with neurologists or geriatricians.
- Physical Therapists (PT): Focuses on maintaining strength, flexibility, and coordination to preserve mobility and prevent falls—a critical risk as spatial perception changes. We utilize muscle memory and repetitive training to help patients retain the ability to walk, stand, and transfer safely, even as short-term memory declines.
- Occupational Therapists (OT): Provides expertise in dementia adaptation, our OTs modify tasks and the home environment to match the patient’s cognitive abilities, maximizing their success in daily living.
- Speech-Language Pathologists (SLP): Assesses and treats swallowing difficulties (dysphagia) often associated with advanced dementia, and develop communication boards or strategies to help patients express their needs.
- Medical Social Workers: Assists families in planning for the long term, including navigating legal resources, community support groups, and future care transitions.
