Northwest Home Health and Rehab specializes in providing care and guidance to help ease patients from their diagnosis through the later stages of their disease. We provide care for patients in all stages of their disease process including: early, mid and late.

Clinicians keep in close communication with facilities, physicians, caregivers and families to keep everyone in the care team involved during the care planning process. Our clinicians are highly skilled in memory care and complete annual Alzheimer’s and Dementia training in order to provide the best care for our patients. We offer:
