Northwest Home Health and Rehab provides home health care services in the greater Chicago area, including Boone, Cook, DeKalb, DuPage, Kane, Kendall, Lake, McHenry, Ogle, Will and Winnebago counties.
Located northwest of Chicago, we have strong relationships with many well-known hospital systems, teaching hospitals and nationally recognized orthopedic groups.
We are a Medicare-certified home health agency licensed by the State of Illinois Department of Public Health to serve the shaded areas in the map above. Some of these service areas include:
