Northwest Home Health & Rehab’s office is available 24/7/365 to receive referrals and address patient questions and concerns. Referrals may be called in, faxed or sent through Careport and AIDA.

Download and fill out our fast track referral form. Fax this information to Intake at 847-854-0213. We will verify insurance and staffing before notifying you if we can accept the patient.
