Our Services

Northwest Home Health & Rehab employs a group of highly-trained nurses and therapists to create a team that will best fit the recovery needs of the patient. On average, our care providers have about 15 years of experience, and many have specialty backgrounds, such as cardiac care or memory care.

Many of our team members began their careers in hospitals and physician’s offices and understand that each physician has different protocols. Our staff strictly abides by all physician, state and Medicare guidelines to ensure agency compliance and the best possible outcome for the patient.

Northwest Home Health & Rehab’s professional services include:

  • Registered Nurses (RN): Our RNs and Case Managers manage the entire plan of care with a focus on high-acuity clinical needs. They are proficient in management of peripherally inserted central catheter (PICC) and Central Line maintenance and Complex Urological Care, providing expert management of indwelling, suprapubic catheters. Additionally, our nursing team includes Wound Care Specialists trained in evidence-based protocols for surgical incisions and chronic ulcers, utilizing advanced modalities to accelerate healing and prevent re-hospitalization.

  • Physical Therapists (PT): Our PTs work with patients to improve strength, stability, and endurance. They utilize specialized rehabilitation techniques to assist with a wide array of therapy needs, from general deconditioning to Total Joint Replacement recovery, helping patients reach their highest level of physical function and safety within their home environment.

  • Occupational Therapists (OT): Our OTs concentrate on improving function to enhance daily living skills. They provide expert guidance on adaptive equipment and environmental modifications, focusing on achieving maximum functional independence and safety during routine daily activities.

  • Medical Social Workers (MSW): Our MSWs provide essential support by assessing social and emotional factors related to a patient’s illness. They assist with long-term planning, counseling, and referrals to community resources, ensuring a comprehensive support network is in place.

  • Speech-Language Pathologists (SLP): Our SLPs focus on improving speech, cognition, and general language function. They also provide critical evaluations and treatment for swallowing disorders (dysphagia), helping patients regain vital communication and nutritional safety.

  • Home Health Aides (HHA): Working under the direct supervision of a licensed clinician, our aides assist with various types of personal care and
